Assessment services are limited to students in Utah.

A parent/guardian with legal custody must agree to the assessment.

If your student will need ADA accomodations, you will need to contact the OEO/AA before signing up for an assessment time.

Your assessment slot will not be reserved if these conditions are not met.

If your child receives free/reduced lunch, or if your family receives Medicaid or Food Stamps, your child may qualify for one of a limited number of free assessments.
